I've had 2 cleanings/exams in 2014, as allowed in my statement of benefits. Both should be covered at 100%. UC has now denied the claim twice saying that it exceeds the 2 cleanings/exams allowed in a twelve month period. What they are doing is crossing calendar years and counting back to Dec 2013 as the first exam!

Yet the benefit period began on Jan 1, 2014, per their own plan.

Unbelievable. I also had 2 crowns during the year, and it took them 4 months with repeated rejected claims before they'd finally paid their portion of it. I feel sorry for my dentist office in having to deal with them. I should have taken that as a huge sign and switched carriers. Nothing but trouble from them when it comes to actually getting the benefits you've paid for with your premium.
